Assistive Technology forms
The Ministry of Education provides assistive technology services that support and guide staff and specialists working directly with students.

Assistive Technology application form 2010
Use this form for any new request for assistive technology funding. The 2010 version replaces all previous versions and incorporates new priority criteria. A help document is also available if you need any support or further information to complete your application.

Loan form
This form is used to record responsibilities for loan technology that is used as part of an Assistive Technology trial.
Transfer form
This form is used to document the transfer of technology and of school responsibilities when a student moves from one school to another.
Variation form
This form is used when there is a change of pricing after original quote or technology is no longer available or a change of circumstances requires minor change within three months of the original Assistive Technology allocation.
Replacement or repair form
This form is used when technology needs to be replaced or repaired. If there is a change in student circumstances and an upgrade or change to technology is necessary, then a full application needs to be completed using the Application Form.
Training plan form
This form is used to document a training plan for either technology being currently applied for or when there is a change in the student's circumstances and training is required for new staff.
Assistive Technology: Management plan template
This template is used by the school based team to outline how the technology will be cared for at school and during transitions. It is common in cases where there are risks that the equipment may be damaged, stolen or lost.
Review form
This form is used when reviewing the effectiveness of the Assistive Technology allocated by the Ministry of Education.
Assistive Technology joint funding form (Education and Health)
This form is used to apply for equipment/assistive technology identified as meeting a student’s essential living and learning needs. The identified solution must be a total value of $5,000 or more (including GST). Please refer to the Joint Funding fact sheet [PDF; 358kb] for more information.
